MRO accepts third-party requests and provider requests for Arizona hospitals and clinics except Page and Payson. For continuation of care at Western Region and Arizona rural facilities, follow Banner’s separate HIMS instructions. Patients requesting their own records should use the patient-request process.
Send the authorization and record details required for your requester type. Specify the Banner facility and care dates. If someone other than the patient signs, provide the authority required by Banner’s instructions.
Name the receiving department and the hospital, clinic or service involved. Add the document title, page count and a callback number. Put patient identifiers and authorization details in the fields the official form requests; a cover page does not replace signed consent.

Keep the transmission result and a copy of the documents. Confirm with the receiving department that the packet is complete and can be processed; a delivery receipt does not establish acceptance or a decision.
