Choose the region where you received care, then the medical center or records service. Washington image requests are separate from written records. Mid-Atlantic requests go to the named center’s Health Information Management office.
Use your region’s authorization to disclose health information. Name the patient, medical record number, treatment dates and requested records. Disability and FMLA paperwork must follow the regional medical-request instructions.
Name the receiving department and the hospital, clinic or service involved. Add the document title, page count and a callback number. Put patient identifiers and authorization details in the fields the official form requests; a cover page does not replace signed consent.
