Written medical records use the centralized Release of Information fax. Copies of radiology images or films use the treating hospital’s imaging line below. An imaging report is not the same as the underlying images.
For written records, complete a records authorization and identify the facilities, treatment dates and record types. For images or films, use the hospital’s imaging instructions and signed release. Chart corrections use the separate amendment form.
Name the receiving department and the hospital, clinic or service involved. Add the document title, page count and a callback number. Put patient identifiers and authorization details in the fields the official form requests; a cover page does not replace signed consent.

Keep the transmission result and a copy of the documents. Confirm with the receiving department that the packet is complete and can be processed; a delivery receipt does not establish acceptance or a decision. These are fax destinations, not voice support lines. Check with Mass General Brigham before sending a duplicate.
