Use the Release of Information Team destination for the completed records form. If an accessible format is needed, tell the office when submitting the request. Referrals and new appointments need the clinical team’s instructions.
Complete the appropriate medical release form with treatment dates, records and recipient details. State any accessible-format needs and include the signature or representative authority the form requires.
Name the receiving department and the hospital, clinic or service involved. Add the document title, page count and a callback number. Put patient identifiers and authorization details in the fields the official form requests; a cover page does not replace signed consent.
