A coverage determination asks the plan to cover a drug or grant an exception. A redetermination appeals a coverage decision already made. These numbers are published for the CalPERS Medicare material linked here; compare your own plan’s form before using them.
For a coverage request, use the prior authorization or coverage form with the prescriber’s supporting information. For an appeal, identify the denial and explain why it should be reconsidered, with relevant evidence.
Name the receiving department and the hospital, clinic or service involved. Add the document title, page count and a callback number. Put patient identifiers and authorization details in the fields the official form requests; a cover page does not replace signed consent.
