Unum’s employee support page gives a general claims-information destination. Its individual disability packet specifies a separate fax. Use the return instructions on your claim form when they differ.
For individual disability, complete the claimant statement, occupation information and authorizations requested in the packet. The treating physician completes the attending physician statement.
